2. To receive your rebate you must mail the original Mail-In Rebate Form with the required supporting documents to the address listed above, rebate requests must be postmarked on or before 7/15/2007 and received by 7/30/2007.
3. Limit one rebate per home.
4. Offer good only in North America (U.S. and Canada) and void where prohibited and void where prohibited, taxed or restricted by law.
5. Zoe Access Canopy installation cannot be refunded once this rebate form has been submitted.
